Overview

This is a combination regimen consisting of three agents: - **Olaparib** is a small molecule inhibitor of poly (ADP-ribose) polymerase (PARP), an enzyme involved in DNA repair. By inhibiting PARP, olaparib prevents cancer cells from repairing damaged DNA, leading to cell death, particularly in tumors with defects in homologous recombination repair such as BRCA mutations. - **Paclitaxel** is a microtubule-stabilizing agent that inhibits cell division by promoting the assembly and stabilization of microtubules, thereby preventing their disassembly which is necessary for mitosis. - **Pembrolizumab** is a humanized monoclonal antibody targeting the programmed death 1 (PD-1) receptor on T cells. By blocking PD-1 interaction with its ligands PD-L1 and PD-L2, pembrolizumab enhances immune-mediated tumor cell killing. The combination aims to exploit complementary mechanisms—DNA damage induction and impaired repair (olaparib), cytotoxicity via mitotic arrest (paclitaxel), and immune checkpoint inhibition to enhance antitumor immunity (pembrolizumab). This regimen has been investigated primarily in advanced solid tumors including gastric cancer[8], ovarian cancer[4][9], non-small cell lung cancer[3], and other molecularly selected cancers[2]. Early-phase trials have shown initial antitumor activity with manageable safety profiles.
