Olaptesed pegol + radiotherapy + pembrolizumab is a combination regimen under investigation primarily for glioblastoma. Olaptesed pegol (NOX-A12) is a PEGylated L-stereoisomer RNA aptamer that binds and inhibits CXCL12, disrupting interactions that support tumor cell proliferation, angiogenesis, and metastasis. Pembrolizumab (Keytruda) is a humanized monoclonal antibody targeting PD-1, blocking its interaction with PD-L1 and PD-L2 to enhance anti-tumor immune responses. Radiotherapy uses ionizing radiation to damage or kill cancer cells. This specific combination is being studied in the phase 1/2 GLORIA trial for newly diagnosed, unmethylated MGMT promoter glioblastoma, where olaptesed pegol and pembrolizumab are administered concurrently with radiotherapy to boost immune response and potentially improve survival[3][5].
