Overview

This drug is a fixed-dose combination of three antihypertensive agents: - **Olmesartan medoxomil** is an angiotensin II receptor blocker (ARB) that selectively blocks the angiotensin II type 1 receptor, leading to vasodilation and reduced blood pressure. - **Amlodipine** is a dihydropyridine calcium channel blocker (CCB) that inhibits the influx of calcium ions into vascular smooth muscle and cardiac muscle, resulting in relaxation of blood vessels and decreased peripheral resistance. - **Azelnidipine** is also a dihydropyridine CCB with long-lasting effects on blood pressure control. It provides persistent antihypertensive action, particularly effective for early morning hypertension. The combination targets multiple mechanisms involved in hypertension—blocking the renin–angiotensin system via olmesartan and inhibiting calcium-mediated vascular contraction via two different CCBs. This multi-pronged approach can provide more effective blood pressure reduction than monotherapy or dual therapy alone. The combination may also offer benefits such as improved endothelial function, reduced sympathetic activity, antioxidant effects in the brainstem, renal protection (especially relevant for patients with diabetes), and better adherence due to single-tablet regimens[1][3][4][5].
