Olmesartan medoxomil + calcium channel blocker (CCB) is a combination antihypertensive therapy developed by Daiichi Sankyo. It consists of olmesartan medoxomil, a prodrug that is hydrolyzed to olmesartan, a selective angiotensin II type 1 (AT1) receptor antagonist, and a dihydropyridine calcium channel blocker (typically amlodipine). Olmesartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II by selectively binding to the AT1 receptor in vascular smooth muscle and the adrenal gland. The CCB component inhibits the transmembrane influx of calcium ions into vascular smooth muscle and cardiac muscle, resulting in vasodilation and decreased peripheral vascular resistance. This combination is primarily indicated for the treatment of essential hypertension, particularly in patients whose blood pressure is not adequately controlled by monotherapy, providing additive blood pressure lowering through complementary mechanisms of action.
