Overview

Olmesartan medoxomil + hydrochlorothiazide + amlodipine + hydralazine is a combination of four antihypertensive agents, each with a distinct mechanism of action. Olmesartan medoxomil is an angiotensin II receptor blocker (ARB) that inhibits the vasoconstrictive effects of angiotensin II, leading to vasodilation and reduced blood pressure. Hydrochlorothiazide is a thiazide diuretic that lowers blood pressure by increasing renal excretion of sodium and water. Amlodipine is a calcium channel blocker that relaxes vascular smooth muscle by inhibiting calcium influx, resulting in decreased peripheral resistance and lower blood pressure. Hydralazine is a direct-acting vasodilator that primarily relaxes arteriolar smooth muscle, further reducing systemic vascular resistance and lowering blood pressure. This combination may be considered for patients with resistant hypertension or when multiple mechanisms are needed to achieve adequate blood pressure control.
