Olmesartan medoxomil + thiazide diuretic refers to a combination antihypertensive regimen evaluated in the COLM (Combination of OLMesartan and a calcium channel blocker or diuretic in Japanese elderly hypertensive patients) Study. Olmesartan medoxomil is a small molecule angiotensin II receptor blocker (ARB) that selectively blocks the AT1 receptor, thereby inhibiting the vasoconstrictive and aldosterone-secreting effects of angiotensin II. The thiazide-type diuretic component—which in the COLM trial included low doses of trichlormethiazide (≤1 mg), hydrochlorothiazide (≤12.5 mg), or indapamide (≤1 mg)—acts by inhibiting the sodium-chloride symporter (NCC) in the distal convoluted tubule of the kidney. This inhibition leads to increased excretion of sodium and water, reducing extracellular fluid volume and blood pressure. The COLM Study Research Organization conducted a Phase 4, investigator-initiated trial to compare this combination's efficacy in reducing cardiovascular morbidity and mortality against a combination of olmesartan and calcium channel blockers in high-risk elderly Japanese patients.
