Overview

The ABC regimen is an experimental, chemotherapy-free combination therapy being evaluated for the treatment of newly diagnosed Philadelphia chromosome-positive (Ph+) acute lymphoblastic leukemia (ALL). This triplet regimen consists of **olverembatinib** (a third-generation BCR-ABL1 tyrosine kinase inhibitor), **blinatumomab** (a CD3/CD19 bispecific T-cell engager), and **chidamide** (a subtype-selective histone deacetylase inhibitor). Olverembatinib is designed to overcome resistance mutations, including the T315I mutation, which often confers resistance to earlier-generation TKIs. Blinatumomab facilitates a T-cell-mediated immune response against CD19-expressing leukemia cells, while chidamide acts as an epigenetic modulator to potentially enhance the efficacy of the other agents and overcome further resistance mechanisms. The regimen is being studied primarily by researchers at Nanfang Hospital, Southern Medical University, as a first-line treatment option for patients, including those ineligible for allogeneic hematopoietic stem cell transplantation.
