Overview

Omacetaxine mepesuccinate + azacitidine is a combination therapy being investigated by the University of Colorado, Denver, for the treatment of high-grade myelodysplastic syndromes (MDS) and chronic myelomonocytic leukemia (CMML). Omacetaxine mepesuccinate is a semisynthetic form of homoharringtonine that acts as a protein synthesis inhibitor by binding to the ribosomal A-site, thereby preventing the initial step of protein translation. This leads to the downregulation of short-lived oncoproteins like MCL-1, which are critical for the survival of leukemic stem cells. Azacitidine is a hypomethylating agent that incorporates into DNA and RNA, inhibiting DNA methyltransferase and leading to DNA hypomethylation and direct cytotoxicity. The combination aims to synergistically target the protein synthesis machinery and epigenetic regulation in myeloid malignancies.
