Overview

**Omaveloxolone + nivolumab** is an investigational combination therapy being evaluated primarily for unresectable or metastatic melanoma and other advanced solid tumors. **Omaveloxolone** is a second-generation synthetic oleanane triterpenoid that activates Nrf2, leading to upregulation of antioxidant and cytoprotective genes, targeting oxidative stress and inflammatory pathways[4][5]. **Nivolumab** is a monoclonal antibody checkpoint inhibitor that blocks PD-1, enhancing T cell–mediated immune responses against tumors[2][4][6]. The rationale for combining them is to potentially synergize immunomodulation (by reducing myeloid-derived suppressor cell–mediated immunosuppression with omaveloxolone) and immune checkpoint inhibition (with nivolumab) for improved antitumor activity[4]. This combination is under clinical investigation and is not approved for use outside of trials.
