Omecamtiv mecarbil is a first-in-class, small molecule cardiac myosin activator developed for the treatment of heart failure with reduced ejection fraction (HFrEF). It works by directly targeting and activating the contractile mechanism of cardiac muscle, specifically binding to the catalytic S1 domain of cardiac myosin. This action increases the number and duration of active actin-myosin cross bridges during each cardiac cycle, thereby augmenting impaired contractility associated with HFrEF. Omecamtiv mecarbil enhances force production in heart muscle without increasing intracellular calcium or myocardial oxygen consumption, leading to improved systolic function and increased left ventricular ejection time and stroke volume. The drug is being developed by Cytokinetics.
