Omega-3-acid ethyl esters is a prescription lipid-regulating agent composed of a highly concentrated mixture of ethyl esters of omega-3 fatty acids, primarily eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA). It is FDA-approved as an adjunct to diet to reduce triglyceride levels in adult patients with severe hypertriglyceridemia (triglycerides ≥ 500 mg/dL). The drug's mechanism of action involves the inhibition of hepatic enzymes such as acyl-CoA:1,2-diacylglycerol acyltransferase (DGAT), which reduces the synthesis of triglycerides, and the activation of peroxisome proliferator-activated receptor alpha (PPAR-alpha), which increases fatty acid oxidation. These effects lead to a decrease in the production and secretion of very-low-density lipoprotein (VLDL) particles from the liver. Originally developed by Pronova BioPharma and marketed as Lovaza by GSK, it is now widely available as a generic medication.
