Omega-3 fatty acids, primarily eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), are long-chain polyunsaturated fatty acids that modulate inflammatory and thrombotic pathways. Shiraz University of Medical Sciences investigated the efficacy of a 3g loading dose of omega-3 fatty acids administered 12 hours prior to elective percutaneous coronary intervention (PCI) in patients with coronary arteriosclerosis. This pretreatment strategy aims to reduce procedural myocardial injury and major adverse cardiac events (MACE) by stabilizing atherosclerotic plaques, improving endothelial function, and reducing the systemic inflammatory response associated with the procedure. The fatty acids act as ligands for nuclear receptors like PPAR-alpha and cell surface receptors like GPR120, and they competitively inhibit the conversion of arachidonic acid into pro-inflammatory eicosanoids.
