Omega-3 polyunsaturated fatty acids (PUFAs) are being investigated as an intravenous immunonutrition therapy for patients with acute-on-chronic liver failure (ACLF). In clinical research conducted by the Institute of Liver and Biliary Sciences (ILBS) in India, intravenous omega-3 lipid emulsions (specifically the 10% fish oil emulsion Omegavan) are used to modulate the systemic inflammatory response and immune dysfunction characteristic of ACLF. The treatment aims to reduce systemic inflammation, endotoxemia, and the incidence of bacterial sepsis by competing with arachidonic acid for the production of less inflammatory eicosanoids and promoting the synthesis of specialized pro-resolving mediators (SPMs). Clinical trial results have indicated that this intervention is safe and effective in improving clinical outcomes and potentially reducing 28-day mortality in ACLF patients.
