This quadruple allopathic regimen is a combination therapy used for the eradication of Helicobacter pylori infection. It consists of a proton pump inhibitor (omeprazole), two antibiotics (amoxicillin and metronidazole), and a bismuth compound. Omeprazole reduces gastric acid secretion by covalently inhibiting the H+/K+-ATPase pump in gastric parietal cells, creating an environment that enhances the activity of the co-administered antibiotics. Amoxicillin and metronidazole provide multi-targeted antibacterial action against H. pylori, while the bismuth compound exerts direct antimicrobial effects and provides mucosal protection. This specific regimen (20 mg omeprazole, 1g amoxicillin, 500 mg metronidazole, and 400 mg bismuth compound for 7 days) was evaluated in a Phase 2/3 clinical trial in Pakistan led by Hafiz Muhammad Asif, where it demonstrated an eradication rate of approximately 62.2% in an intention-to-treat analysis.
