Omeprazole + perindopril is a therapeutic combination consisting of a proton pump inhibitor (PPI) and an angiotensin-converting enzyme (ACE) inhibitor. Omeprazole acts by irreversibly inhibiting the gastric H+/K+-ATPase enzyme system (the proton pump) in the parietal cells of the stomach, effectively suppressing gastric acid secretion. This is utilized in the management of gastroesophageal reflux disease (GERD), peptic ulcers, and for gastric protection. Perindopril is a long-acting ACE inhibitor that is converted into its active metabolite, perindoprilat, which prevents the conversion of angiotensin I to angiotensin II. This results in decreased systemic vascular resistance, lower blood pressure, and reduced afterload, making it a mainstay in the treatment of hypertension, heart failure, and stable coronary artery disease.
