MET409 (omesdafexor) is a potent and selective small molecule agonist of the farnesoid X receptor (FXR), a nuclear receptor highly expressed in the liver and intestinal tract. Developed by Metacrine for oral administration as a once-daily therapy, it is designed to treat non-alcoholic steatohepatitis (NASH) and other liver diseases. FXR activation by MET409 regulates bile acid metabolism and has anti-inflammatory and antifibrotic effects. In clinical trials, MET409 demonstrated significant reduction in liver fat content with a favorable safety profile compared to other FXR agonists. The drug does not cross-react with TGR5 or other related receptors. It has received FDA Fast Track designation for NASH due to its differentiated therapeutic profile[1][4][5][6].
