OMKO1 is a lytic Pseudomonas aeruginosa–specific bacteriophage developed as a therapeutic agent to both kill multidrug-resistant P. aeruginosa and evolutionarily “steer” the bacteria toward loss of antibiotic resistance. It binds components of the Mex efflux pump complex, particularly the outer membrane protein OprM, so that mutations conferring phage resistance disrupt efflux function and resensitize bacteria to multiple antibiotics such as ceftazidime, ciprofloxacin, tetracycline, erythromycin, and others.[7][9] In vitro and in vivo studies show that OMKO1 reduces bacterial load, restores antibiotic susceptibility, and can synergize with antibiotics, and it has been used under compassionate/emergency use and in early clinical trials including aerosolized administration for chronic lung infections (e.g., cystic fibrosis, non-CF bronchiectasis, COPD) caused by P. aeruginosa.[1][3][5][6][8][10]
