OMP-54F28 (ipafricept) is a first-in-class recombinant fusion protein that consists of the extracellular domain of the human frizzled 8 receptor fused to a human IgG1 Fc fragment. It acts as a decoy receptor, binding Wnt ligands and thereby blocking Wnt signaling, which is implicated in tumor cell de-differentiation and cancer stem cell function. Nab-paclitaxel (nanoparticle albumin-bound paclitaxel, brand name Abraxane) is a microtubule inhibitor formulated with albumin nanoparticles to improve delivery and reduce toxicity compared to conventional paclitaxel. Gemcitabine (brand name Gemzar) is an antimetabolite chemotherapy agent that inhibits DNA synthesis by acting as a nucleoside analog. The combination of these three agents has been investigated in clinical trials for advanced solid tumors, particularly pancreatic cancer, aiming to target both bulk tumor cells and cancer stem cells through complementary mechanisms: inhibition of Wnt signaling by OMP-54F28/ipafricept, disruption of microtubules by nab-paclitaxel, and inhibition of DNA synthesis by gemcitabine[1][4][5][6][7].
