OMP-54F28 (ipafricept) is a first-in-class recombinant fusion protein that combines the extracellular cysteine-rich domain of human frizzled family receptor 8 (Fzd8) with the Fc region of human IgG1. It acts as a decoy receptor, binding Wnt ligands and thereby antagonizing Wnt signaling, which is implicated in tumor cell dedifferentiation, cancer stem cell function, and tumor progression. In preclinical models and early-phase clinical trials, OMP-54F28 has demonstrated inhibition of tumor growth and reduction in cancer stem cell frequency both as a single agent and in combination with chemotherapeutic agents such as paclitaxel and carboplatin. The combination has been studied particularly in recurrent platinum-sensitive ovarian cancer to determine safety, tolerability, pharmacokinetics, immunogenicity, dose-limiting toxicities (DLTs), maximum tolerated dose (MTD), recommended phase 2 dose (RP2D), and preliminary efficacy[1][2][3][6][8].
