OMT-28 is a fully synthetic small molecule drug designed as an analog of the natural omega-3 fatty acid metabolite 17,18-epoxyeicosatetraenoic acid (17,18-EEQ). Developed by OMEICOS Therapeutics, it mimics the structure and function of omega-3 epoxyeicosanoids to provide cardioprotective and anti-inflammatory effects. Mechanistically, OMT-28 acts as an agonist at peroxisome proliferator–activated receptor alpha (PPARα), modulates phosphatidylinositol 3 kinase gamma (PI3Kγ), stimulates sirtuin activity, and influences nitric oxide signaling and protein kinase pathways. Preclinical studies show that it improves mitochondrial function in cardiomyocytes, reduces pro-inflammatory signaling, preserves cell viability under hypoxic stress, limits inflammasome activation after ischemia/reperfusion injury in heart tissue, and reduces markers of inflammation such as TNFα and NF-kB activity. Clinically, it is being investigated for atrial fibrillation maintenance post-cardioversion (PROMISE-AF trial), primary mitochondrial disease with myopathy/cardiomyopathy (PMD OPTION study), cardiomyopathies, musculoskeletal disorders with inflammation, and wet age-related macular degeneration[1][2][4][5][7][8].
