**OMT-28** is an investigational, orally administered small-molecule analog of the omega-3 fatty-acid metabolite 17,18-epoxyeicosatetraenoic acid. Developed by OMEICOS Therapeutics, it is designed to reduce mitochondrial dysfunction, oxidative stress, and inflammation through biased modulation of sphingosine-1-phosphate receptor 1 signaling and activation of the AMPK/SIRT1/PGC-1alpha mitochondrial-restoration axis. OMT-28 has completed a Phase 2a study in primary mitochondrial disease with mitochondrial myopathy and/or cardiomyopathy, and OMEICOS has announced plans to advance the program into a pivotal Phase 3 study.
