OMTX705 is a first-in-class antibody-drug conjugate (ADC) developed to target the tumor microenvironment in advanced solid tumors. It specifically binds to fibroblast activation protein alpha (FAPα), which is highly expressed on cancer-associated fibroblasts (CAFs) that play a key role in tumor progression and resistance to therapy. The drug consists of a humanized anti-FAP antibody linked to a novel cytolysin payload. Its dual mechanism of action includes direct cytotoxicity via tubulin inhibition and modulation of the tumor stroma, enhancing immune cell infiltration and overcoming resistance to immunotherapy. Preclinical studies have demonstrated potent antitumor activity as both monotherapy and in combination with chemotherapy or immunotherapy, including efficacy in models resistant to PD-1 inhibitors.
