OMTX705 + nab-paclitaxel + gemcitabine is an investigational combination therapy being developed by Oncomatryx Biopharma S.L. for the treatment of advanced or metastatic pancreatic ductal adenocarcinoma (PDAC). The regimen's novel component, OMTX705, is a first-in-class antibody-drug conjugate (ADC) consisting of a humanized monoclonal antibody targeting Fibroblast Activation Protein (FAP) conjugated to TAM470, a potent cytolysin payload that acts as a tubulin inhibitor. FAP is highly expressed on cancer-associated fibroblasts (CAFs) within the tumor microenvironment, which are known to create a physical and biochemical barrier against drug delivery and immune infiltration. By targeting CAFs, OMTX705 aims to disrupt the tumor stroma, while the standard-of-care chemotherapy doublet of nab-paclitaxel (an albumin-bound microtubule stabilizer) and gemcitabine (a nucleoside analog that inhibits DNA synthesis) directly targets the malignant epithelial cells. This dual-action approach is currently being evaluated in Phase 1b clinical trials, sometimes in further combination with the PD-1 inhibitor tislelizumab.
