Overview

onabotulinumtoxinA + dimethyl sulfoxide (DMSO) is an experimental combination therapy being evaluated by the Mayo Clinic for the treatment of refractory overactive bladder (OAB) and detrusor overactivity. This novel approach utilizes intravesical instillation (bladder instillation) as a non-invasive alternative to the standard FDA-approved method of intradetrusor injection. OnabotulinumtoxinA is a botulinum neurotoxin type A that acts by cleaving the SNAP-25 protein, which inhibits the presynaptic release of acetylcholine at the neuromuscular junction, leading to relaxation of the detrusor muscle. Dimethyl sulfoxide (DMSO) is included in the formulation as a chemical penetration enhancer to facilitate the transport of the large toxin molecules across the bladder urothelium to reach the underlying nerves and muscle.
