Oncofid-P-B is a bioconjugate consisting of the microtubule-stabilizing cytotoxic agent paclitaxel covalently linked to hyaluronic acid (HA). Developed by Fidia Farmaceutici, it is specifically designed for the intravesical treatment of non-muscle invasive bladder cancer (NMIBC), particularly carcinoma in situ (CIS) in patients who are unresponsive or intolerant to Bacillus Calmette-Guérin (BCG) therapy. The hyaluronic acid component serves as a targeting ligand for the CD44 receptor, which is frequently overexpressed on the surface of bladder cancer cells. This targeting facilitates receptor-mediated endocytosis, allowing for the selective delivery and internalization of paclitaxel into malignant cells while minimizing systemic absorption and associated toxicities. Once internalized, paclitaxel is released to bind to tubulin, inhibiting microtubule disassembly and leading to cell cycle arrest and apoptosis.
