ONCOS-102 + pemetrexed + cisplatin is a combination immunotherapy and chemotherapy regimen developed by Targovax Oy (now Circio Holding ASA) for the treatment of malignant pleural mesothelioma. The primary investigational component, ONCOS-102, is a chimeric Ad5/3 oncolytic adenovirus engineered with a 24-base pair deletion (D24) in the E1A region for tumor-selective replication and a transgene for human granulocyte-macrophage colony-stimulating factor (GM-CSF). This virus selectively infects and lyses cancer cells, releasing tumor antigens and expressing GM-CSF to recruit and activate antigen-presenting cells, thereby inducing a systemic anti-tumor T-cell response. This is combined with the standard-of-care chemotherapy doublet of pemetrexed (a multi-targeted antifolate) and a platinum agent (cisplatin or carboplatin), which inhibits DNA synthesis and induces DNA damage. The combination aims to leverage the immunogenic cell death caused by chemotherapy and the immune-priming effects of the oncolytic virus to overcome the immunosuppressive tumor microenvironment of mesothelioma.
