OncoSil is a minimally invasive, intratumoural brachytherapy device that consists of microparticles loaded with the radioactive isotope phosphorus-32 (32P). It is designed for implantation directly into pancreatic tumours via endoscopic ultrasound-guided injection. The device emits targeted beta radiation over a short distance, enabling high-dose localised radiotherapy with minimal exposure to surrounding healthy tissue. The primary **mechanism of action** is the local emission of beta particles from phosphorus-32, causing direct DNA damage, inhibition of cancer cell division and proliferation, and eventual tumour shrinkage. OncoSil is primarily indicated for **locally advanced unresectable pancreatic cancer** in combination with gemcitabine-based chemotherapy. CE-marked in the EU and UK, OncoSil is supported by multiple clinical trials that showed promising local disease control and the potential to downstage unresectable disease to operable status.
