ondansetron + topiramate

Ondansetron + topiramate is a fixed-dose combination of two small molecule drugs—ondansetron, a selective serotonin 5-HT3 receptor antagonist, and topiramate, an anticonvulsant that modulates GABAergic and glutamatergic neurotransmission. This combination has been investigated primarily for the treatment of alcohol use disorder (AUD) and alcoholism. Preclinical studies suggest that the combination may be more effective than either agent alone in reducing alcohol consumption and preventing relapse, particularly in heavy drinkers. Ondansetron acts by blocking serotonin 5-HT3 receptors involved in reward pathways, while topiramate modulates GABA-A receptors (agonist), antagonizes AMPA/kainate-type glutamate receptors (GluR-5), inhibits voltage-gated sodium channels, and inhibits carbonic anhydrase II/IV enzymes. The synergistic effect is thought to arise from targeting multiple neural pathways implicated in addiction[1][3][7][8].
