Overview

Onfasprodil (MIJ821) is a small molecule N-methyl-D-aspartate (NMDA) receptor antagonist specifically targeting the GluN2B (NR2B) subunit. Developed by Novartis, it is intended for the rapid treatment of Major Depressive Disorder (MDD), particularly in patients presenting with suicidal ideation and intent. By selectively modulating the GluN2B subunit rather than providing broad NMDA blockade, onfasprodil aims to achieve the rapid-acting antidepressant efficacy seen with ketamine while reducing the incidence of psychotomimetic and dissociative side effects. It is typically administered via intravenous infusion and has been evaluated in Phase 2 clinical trials for its safety and efficacy in reducing depressive symptoms and suicidal intent.
