Overview
This combination drug consists of **onfekafusp alfa (L19TNF, marketed as Fibromun)**, an antibody-cytokine fusion protein, and **lomustine**, an alkylating chemotherapy agent. Onfekafusp alfa targets the extra-domain B (ED-B) of fibronectin found on tumor vasculature and delivers tumor necrosis factor alpha (TNF-α) directly to cancer sites, stimulating anti-tumor immune responses and converting "cold" tumors into "hot" (immunologically responsive) ones. Lomustine is a DNA-alkylating agent that induces tumor cell death via DNA damage. The combination is being investigated for **recurrent glioblastoma**, where preclinical and early clinical results suggest synergistic anti-tumor efficacy and durable responses, likely due to increased immunogenicity and tumor necrosis. The combination is under development and clinical trial investigation by Philogen.
