ONO-2160 is an oral ester-type prodrug of levodopa (L-DOPA) developed by Ono Pharmaceutical for the treatment of Parkinson's disease. It was designed to provide more stable plasma levodopa concentrations and reduce the motor fluctuations (such as "off" periods) associated with standard levodopa therapy. ONO-2160 features a unique metabolic activation pathway in human plasma: it is first hydrolyzed by alpha-1-acid glycoprotein (AGP) into an intermediate metabolite, ONO-M, which is then rapidly converted into active levodopa by human serum albumin (HSA). This relay-type enzymatic reaction allows for a controlled release of levodopa into the systemic circulation. Clinical studies have evaluated ONO-2160 in combination with carbidopa, demonstrating its potential to maintain effective levodopa levels and improve motor function in patients.
