ONO-2909 is an investigational, orally active small molecule orexin 2 receptor (OX2R) agonist being developed by Ono Pharmaceutical for the treatment of narcolepsy. Narcolepsy, particularly type 1, is a chronic neurological disorder characterized by the loss of orexin-producing neurons in the hypothalamus, which leads to excessive daytime sleepiness and cataplexy. By acting as a selective agonist at the OX2R, ONO-2909 is designed to mimic the signaling of endogenous orexin (hypocretin), thereby promoting wakefulness and stabilizing sleep-wake cycles. The drug is currently in Phase I clinical development, where it is being evaluated for safety, tolerability, pharmacokinetics, and cerebrospinal fluid penetration in healthy volunteers (including Japanese and Caucasian populations) and patients with narcolepsy.
