The Onyx Liquid Embolic System is a non-adhesive liquid embolic agent used in interventional neuroradiology for the occlusion of vascular malformations and aneurysms. Developed by Medtronic (originally Micro Therapeutics Inc.), the system consists of an ethylene vinyl alcohol (EVOH) copolymer dissolved in dimethyl sulfoxide (DMSO), with micronized tantalum powder added to provide radiopacity for fluoroscopic visualization. Upon delivery through a microcatheter into the target vessel, the DMSO solvent rapidly diffuses into the blood, causing the EVOH copolymer to precipitate and form a solid, spongy, coherent embolus. This mechanism provides durable occlusion of abnormal vascular structures such as brain arteriovenous malformations (bAVMs), dural arteriovenous fistulas, and intracranial aneurysms. It is available in different viscosities (Onyx 18, 34, and HD-500) tailored for specific clinical applications.
