OP-145 is a synthetic, 24-amino acid antimicrobial peptide (AMP) derived from the human cathelicidin LL-37, specifically residues 13-36. It was engineered to favor the formation of an amphipathic helix, which enhances its stability and ability to interact with bacterial membranes. Developed initially by the University of Leiden and later progressed by OctoPlus (now part of Dr. Reddy's), OP-145 functions as both a direct antimicrobial agent and an immunostimulant. Its primary mechanism of action involves the disruption of bacterial membrane phospholipids, leading to cell lysis. It has demonstrated potent activity against a range of pathogens, including methicillin-resistant *Staphylococcus aureus* (MRSA). Clinical development was primarily focused on the treatment of chronic suppurative otitis media (CSOM), although its global R&D status is currently listed as discontinued.
