OPA-DC is an autologous dendritic cell (DC) vaccine investigated for the treatment of advanced colorectal cancer (CRC). The vaccine is composed of mature dendritic cells loaded with carcinoembryonic antigen (CEA) peptides. The maturation process utilizes a specific cocktail abbreviated as OPA, which consists of OK432 (a lyophilized preparation of Streptococcus pyogenes), a prostanoid (typically prostaglandin E2), and interferon-alpha (IFN-α). This combination is designed to rapidly generate highly potent, mature DCs capable of inducing robust CEA-specific cytotoxic T lymphocyte (CTL) responses and enhancing natural killer (NK) cell activity. Clinical studies have demonstrated that the vaccine is well-tolerated and can stimulate immune responses, although clinical efficacy as a monotherapy in advanced disease has been limited, suggesting a need for additional modulation to achieve significant clinical impact.
