Overview

Opakalim (BHV-7000) is a selective small molecule activator of the Kv7.2/Kv7.3 (KCNQ2/KCNQ3) potassium channels. Developed by Biohaven Pharmaceuticals, it is designed to stabilize neuronal membrane potential and reduce hyperexcitability, which is a key driver in seizure activity and mood disorders. The drug is currently in Phase 3 clinical development for focal epilepsy and idiopathic generalized epilepsy. While it was previously evaluated for major depressive disorder (MDD), a Phase 2 study failed to meet its primary endpoint, leading the company to focus its resources on epilepsy and other priority areas such as immunology and obesity.
