**Opicapone + repaglinide** is a *combination* of two small molecule pharmaceuticals, not marketed together as a fixed-dose combination but sometimes co-administered. Opicapone is a *peripheral, selective, and reversible catechol-O-methyltransferase (COMT) inhibitor*, used as an adjunct to levodopa preparations in Parkinson’s disease to increase levodopa availability by blocking its peripheral degradation[1][4]. Repaglinide is a *short-acting insulin secretagogue* from the meglitinide class, used to control blood glucose in type 2 diabetes by stimulating rapid, glucose-dependent insulin release from pancreatic beta cells[8]. While opicapone may weakly inhibit the CYP2C8 enzyme that metabolizes repaglinide, clinical studies have found no significant pharmacokinetic interaction when both drugs are co-administered[2][5]. No fixed combination product containing both agents exists, and concomitant use is not a recognized or recommended therapy.
