The Opioid-Free Anesthesia-Analgesia (OFA-A) strategy is a multimodal perioperative anesthetic and analgesic protocol developed and studied by the University of Crete. It is designed to mitigate the surgical stress response and avoid the complications associated with perioperative opioid use, particularly in high-risk procedures like elective open abdominal aortic aneurysm (AAA) repair. The strategy involves the co-administration of several non-opioid agents with distinct mechanisms of action, including pregabalin (a gabapentinoid), ketamine (an NMDA receptor antagonist), dexmedetomidine (an alpha-2 adrenergic agonist), lidocaine (a sodium channel blocker), dexamethasone (a corticosteroid), dexketoprofen (an NSAID), paracetamol (an analgesic/antipyretic), and magnesium sulphate (an NMDA antagonist and calcium channel blocker). This combination aims to provide superior or equivalent analgesia while reducing inflammation and the endocrine-metabolic response to surgery.
