The Opioid-free Anesthesia-Analgesia Strategy is a multimodal perioperative pharmacological protocol developed by the University of Crete for patients undergoing elective video-assisted thoracoscopic surgery (VATS) lobectomy for non-small cell lung cancer (NSCLC). This strategy aims to mitigate the immunosuppressive and hyperalgesic effects associated with exogenous opioids by utilizing a diverse combination of non-opioid agents. The protocol incorporates NMDA receptor antagonists (ketamine, magnesium sulphate), alpha-2 adrenergic agonists (dexmedetomidine, clonidine), local anesthetics (lidocaine, ropivacaine), gabapentinoids (pregabalin), and non-steroidal anti-inflammatory drugs (dexketoprofen), alongside standard anesthetic agents like propofol and desflurane. By targeting multiple pain and stress pathways, the strategy seeks to attenuate the surgical stress response and preserve immune function during major thoracic surgery.
