This is an opioid-free anesthesia (OFA) protocol developed by Peking University People's Hospital for use in video-assisted thoracoscopic surgery (VATS) lung resection. The protocol is designed to eliminate the use of intraoperative opioids to reduce postoperative opioid-related adverse effects (ORAEs), such as respiratory depression, nausea, vomiting, and dizziness. It employs a multimodal pharmacological strategy including dexmedetomidine (an alpha-2 adrenergic agonist), lidocaine (a sodium channel blocker), and propofol (a GABA-A receptor modulator), supplemented by regional anesthesia techniques like paravertebral and serratus plane blocks using ropivacaine. This approach aims to provide adequate analgesia and stable hemodynamics while improving the quality of postoperative recovery.
