OPT-821 is a saponin-based immunological adjuvant derived from the soapbark tree Quillaja saponaria, similar to QS-21. It is used as an adjuvant in cancer vaccine formulations, particularly in combination with antigens such as Globo H-KLH for the treatment of various cancers including breast, ovarian, fallopian tube, peritoneal cancer, and neuroblastoma. As an adjuvant, its primary function is to enhance immune responses against co-administered antigens by stimulating both humoral and cellular immunity. OPT-821 has been evaluated in multiple clinical trials as part of investigational vaccine regimens but has not demonstrated significant improvement in progression-free or overall survival when used alone or with polyvalent vaccines compared to control arms[2][3][6]. The compound is administered subcutaneously and has shown a favorable safety profile with injection site reactions being the most common adverse event[4][5].
