The protocol utilizes two partially HLA-matched umbilical cord blood units as the active biological component to provide a source of hematopoietic stem cells for immune and hematopoietic reconstitution. It incorporates an intermediate-intensity conditioning regimen consisting of cyclophosphamide, fludarabine, thiotepa, and total body irradiation (TBI), along with graft-versus-host disease (GVHD) prophylaxis using cyclosporine-A and mycophenolate mofetil. Key optimizations of this protocol include prioritizing cord blood unit quality and CD34+ cell dose over stringent HLA-matching, ensuring therapeutic cyclosporine levels by the day of transplant, and utilizing letermovir prophylaxis for CMV-positive patients to mitigate transplant-related mortality and improve progression-free survival.
