Optimized New Shengmai Powder is a traditional Chinese medicine (TCM) formula developed by the First Teaching Hospital of Tianjin University of Traditional Chinese Medicine specifically for the treatment of chronic heart failure (CHF). It is an enhanced iteration of the classic Shengmai San formula, which traditionally consists of *Panax ginseng*, *Ophiopogon japonicus*, and *Schisandra chinensis*. The 'optimized' version typically incorporates additional botanical agents such as *Astragalus membranaceus*, *Salvia miltiorrhiza*, and *Descurainia sophia* to better address the 'Qi and Yin deficiency' and 'blood stasis' patterns common in heart failure patients. Mechanistically, the formula is designed to improve myocardial energy metabolism, enhance cardiac contractility, and inhibit ventricular remodeling. Clinical studies have demonstrated its ability to reduce levels of N-terminal pro-brain natriuretic peptide (NT-proBNP) and improve the New York Heart Association (NYHA) functional classification in patients with stable chronic heart failure.
