OQL011 is a proprietary nitroglycerin-based topical ointment developed by OnQuality Pharmaceuticals for the treatment of hand-foot skin reaction (HFSR), a common and painful adverse event associated with cancer therapies that inhibit vascular endothelial growth factor receptor (VEGFR) or use tyrosine kinase inhibitors[3][5][7]. HFSR can lead to redness, swelling, pain, and blistering on the palms and soles. There are currently no FDA-approved therapies specifically for HFSR[3][7]. OQL011 works by locally activating VEGF downstream signaling pathways in the skin. The mechanism leverages nitroglycerin as a nitric oxide donor to stimulate soluble guanylate cyclase (sGC), which in turn restores or improves VEGF signaling impaired by VEGFR inhibitors[4][5]. This restoration supports vascular repair mechanisms in areas exposed to high pressure and repetitive force—such as hands and feet—where blood vessels are more susceptible to damage during cancer therapy[5][6][7]. The drug is being evaluated in Phase 2 clinical trials for its safety and efficacy in treating moderate-to-severe VEGFR inhibitor-associated HFSR.
