Oral bacterial lysate (specifically the OM-85 formulation, marketed as Broncho Vaxom) is an immunomodulatory biologic composed of lyophilized chemical lysates from 21 strains of 8 common respiratory pathogenic bacteria. It is designed to stimulate the mucosal immune system and enhance host defense against respiratory tract infections. The mechanism involves the activation of Toll-like receptors (primarily TLR2 and TLR4) on dendritic cells and airway epithelial cells, leading to the maturation of myeloid dendritic cells and the induction of a balanced Th1/Treg immune response. Clinically, it is used to reduce the frequency and severity of recurrent respiratory infections and is currently being evaluated in trials like BLIPA (Bacterial Lysate in Preventing Asthma) to determine its efficacy in preventing persistent wheezing and asthma in infants following severe bronchiolitis.
