Oral sulfate solution (OSS) is an osmotic laxative combination consisting of sodium sulfate, potassium sulfate, and magnesium sulfate, primarily used for bowel cleansing prior to colonoscopy. The mechanism of action involves the creation of an osmotic gradient within the gastrointestinal tract; because the sulfate salts are poorly absorbed, they retain water in the intestinal lumen, increasing the volume and fluidity of the stool to facilitate thorough evacuation. In a Phase III multicenter randomized controlled trial (NCT05465889) led by Changhai Hospital in China, this specific OSS formulation was compared to 3-liter polyethylene glycol (PEG). The study demonstrated that the OSS split-dose regimen was non-inferior to PEG in terms of bowel preparation quality and safety, offering a potentially more convenient alternative for patients due to the lower volume of solution required compared to standard PEG regimens.
