Orelabrutinib + rituximab is a combination therapy consisting of orelabrutinib, a highly selective Bruton tyrosine kinase (BTK) inhibitor, and rituximab, an anti-CD20 monoclonal antibody. Orelabrutinib inhibits BTK signaling in B cells, thereby blocking pathways critical for the survival and proliferation of malignant B cells. Rituximab targets the CD20 antigen on B lymphocytes and mediates cell death through mechanisms such as antibody-dependent cellular cytotoxicity (ADCC), complement-dependent cytotoxicity (CDC), and direct apoptosis induction. The combination has shown synergistic anti-tumor effects in preclinical models by enhancing tumor cell apoptosis while preserving NK-cell-mediated ADCC activity due to the high selectivity of orelabrutinib for BTK over ITK. This regimen is being investigated primarily for relapsed or refractory marginal zone lymphoma (MZL) and other B cell lymphomas[2][3][4].
