Overview

Oreongsan is a traditional East Asian herbal medicine consisting of five ingredients: Polyporus, Alismatis Rhizoma, Atractylodis Rhizoma, Poria, and Cinnamomi Cortex Spissus[1][2][3]. It is primarily prescribed for its diuretic ("isu") effect to regulate water balance and has historically been used for symptoms such as thirst, diminished urination, edema, hangover, and diarrhea[1]. In recent clinical use, Oreongsan is administered as an adjunctive therapy in patients with chronic subdural hematoma (CSDH), particularly to reduce the recurrence of hematoma after surgical intervention[1][2][3]. Its mechanism involves inhibition of aquaporin-4 and aquaporin-1 water channels in the outer membrane of CSDH, preventing fluid inflow into the hematoma[1][2][3][6]. This anti-edematous and anti-inflammatory activity is hypothesized to underlie its clinical effects in both neuro- and cardiovascular indications[4]. Evidence from meta-analyses suggests a statistically significant reduction in CSDH recurrence when Oreongsan is prescribed postoperatively, with a favorable safety profile[3]. The agent is administered orally, typically 2.5–5g, divided into doses per day as per the formulation TJ-17[2][3].
