OrexiCAR T cells are an engineered chimeric antigen receptor (CAR) T-cell therapy developed by CoImmune, Inc. for the treatment of B-cell malignancies. These cells are designed to target CD19 while simultaneously secreting CV1, a high-affinity variant of the SIRPα D1 domain that acts as a checkpoint blocker for the CD47-SIRPα pathway. By locally releasing CV1 at the tumor site, OrexiCAR T cells aim to neutralize the "don't eat me" signal (CD47) on tumor cells, thereby promoting macrophage-mediated phagocytosis (orexigenic activity) and reversing myeloid-mediated immunosuppression in the tumor microenvironment. This dual-action approach is intended to enhance anti-tumor efficacy and overcome resistance mechanisms common in conventional CAR T-cell therapies while minimizing systemic toxicity associated with systemic CD47 blockade.
