Orilanolimab is a humanized, de-immunized monoclonal antibody that acts as an antagonist of the Neonatal Fc receptor (FcRn). By blocking the interaction between FcRn and the Fc portion of IgG molecules, orilanolimab disrupts IgG homeostasis and impedes both innate and adaptive immune functions mediated by pathogenic IgG. This mechanism is intended to reduce levels of circulating pathogenic antibodies in autoimmune diseases. Orilanolimab was originally developed by Syntimmune and later by Alexion AstraZeneca Rare Disease for indications including pemphigus, autoimmune hemolytic anemia, and myasthenia gravis. The drug received orphan designation for pemphigus but its development has been discontinued across all known indications[1][2][3][5][7].
